Where will I serve? Both at the Poverello Health Center and the Food Pantry and Soup Kitchen at the Northside Ministries associated with Assumption Church.
Who will I serve? Clients who utilize the Food Pantry and Soup Kitchen and the patients at the Poverello Health Center.
What will I do? You will engage with people at the Poverello Health Center and Assumption Food Pantry and Soup Kitchen to determine if they have needs beyond food insecurities and access to healthcare. Through your training (informal and formal) you will learn and become familiar with the various resources in the community and help connect individuals to these resources.
You will develop trusting relationships with this population and function as a line between healthcare providers and this population to reduce socioeconomic barriers, provide health advocacy, health education, and navigation services. These services aim to increase health knowledge and self-sufficiency and fall under the umbrella of health prevention services encompassing: Health advocacy and community-based and healthcare referrals, and follow-up to close the loop.
Through conversation and vetted screening tools, you will help identify needs/barriers in this population for the following domains: transportation, financial, employment/job training, education/literacy, food insecurity, childcare, housing, healthcare, government programs, safety, and other socioeconomic barriers not listed.
What will my day look like?
- Build relationships with and welcome people accessing the Food Pantry and Soup Kitchen and the Poverello Health Center.
- During clinic hours interact with healthcare providers and patients.
- Receive training in services available to people in the community/greater community and important information for them to know to navigate the domains listed previously.
